• Required Fields: Your Full Name, Your Email ID, Your Phone Number, Inquiry Subject, and Message.

C. Beneficiary & Support Applications

• Educational Aid Requests: Parents’/Guides’ Name, Student Name, Parents’ Phone Number, Student’s School/University Class, Student’s School UID, and Subject/Aid Request.
• Disability Equipment Requests: Relative’s Name, Patient Name, Relative’s Phone Number, Patient Phone Number, Hospital Name (if admitted/was admitted), and Equipment/Subject Request.
• Clinic Appointments (Homeopathy & Physiotherapy): Patient identity, contact details, appointment slot preferences, and relevant basic clinical intake details required for charitable healthcare delivery.

Purpose of Data Collection & Use

The information collected is utilized exclusively for:

1. Financial & Statutory Compliance: Generating official donation receipts and filing mandatory statutory returns, including Form 10BE tax documentation via government portals.
2. Operational Communication: Responding to inquiries submitted through our contact forms and coordinating welfare disbursements or clinic appointments.
3. Transparency & Reporting: Maintaining internal donor records, accounting audits, and NPO Darpan compliance reporting under Indian laws.
